Certain ingredients in the diet can affect drug metabolism and transport and alter the distribution of the drug in the body. Piperine inhibits drug metabolism enzymes in rodents and increases certain drugs in the human body (including P-glycoprotein substrates phenytoin and rifampicin) The plasma concentration inhibits the transport of digoxin and cyclosporine A on Caco-2 cells; piperine inhibits the drug transporter P-glycoprotein and the drug-metabolizing enzyme CYP3A4, both of which are mainly in intestinal epithelial cells and Hepatocyte expression is the first place for many drugs to be eliminated. Therefore, piperine in food affects the blood concentration of P-glycoprotein and CYP3A4 substrate, especially when taking these drugs orally.
The combination of piperine and nimesulide increases the bioavailability of nimesulide, achieves a better therapeutic index, enhances antinociceptive pain and anti-inflammatory effects, and reduces the frequency of side effects of nimesulide when used alone. The combination of piperine significantly increased the bioavailability of the beta lactam antibiotics amoxicillin and cefotaxime sodium in rats, probably because of the action of piperine on microsomal metabolic enzymes or enzyme systems. In the intravenous injection of phenytoin, the elimination phase of phenytoin was significantly changed in rodents exposed to piperine, and the elimination of phenytoin was also delayed in volunteers taking piperine.
The mechanism by which piperine increases bioavailability is unclear, presumably related to changes in membrane lipid dynamics and the configuration of certain enzymes in the gut, increasing absorption. Piperine increases membrane fluidity and alters enzyme kinetics. Due to the interaction of the non-polar portion of piperine with the lipid and hydrophobic portions of the protein, membrane lipids are reduced as a support for the enzyme protein, thus altering the conformation of the enzyme. Structural studies have shown that piperine increases the length of the endoplasmic reticulum microvilli in intestinal epithelial cells. The authors believe that piperine can induce changes in membrane kinetics and membrane permeation properties, and induce protein synthesis associated with cell scaffold function, resulting in increased intestinal absorption area, assisting substances to effectively penetrate the epithelial cell barrier.
